Following the passage of the health reform law, one of the greatest challenges that will face the American health system and its providers will be the continuing struggle to improve patient safety in hospitals by the elimination of medical error.

In North America it is estimated that as many as 10% of patients admitted to hospital are subject to some form of medical error. Preventable medical errors have been estimated to cause as many as 200,000 deaths annually, with another 500,000 suffering some form of permanent injury. According to the Washington based Institute of Medicine a significant proportion of medical problems in hospital settings are medication errors, resulting in adverse drug events (ADEs), which can be averted by using electronic prescribing combined with decision support tools at point of care. Studies in live North American hospital environments indicate that the application of CPOE technology can reduce these types of errors by more than 50% - and, in some cases, by up to 86%.
